ELIGIBILITY REQUIREMENTS:
All dentists in a practice must be a member in good standing with the TDA and be confirmed as a firm member with TDA to participate. Email the TDA Membership Services Coordinator at grace@tndental.org or call 615-628-0208. Sole proprietors must have a least 1 W-2 employee at 30+ hours a week.
